As the parent/legal guardian of the above child(ren), I authorize any adult acting on behalf of Camp Gan Izzy to secure medical treatment for my child, including hospitalization. I hereby give permission for my child to participate in all camp activities, join in trips on and off camp properties and allow my child to be photographed while participating in Camp Gan Izzy activities. These photos may be used for marketing purposes.
